Just happened to me as well while scanning the downloaded Flac for the new
Nine Inch Nails album. Picard just crashes with a core dump. When trying
to scan the files.

Picard 0.9.0 on Ubuntu Gutsy (from your repository on launchpad).

If I have some time I'll try to look at it closer. Shall I send you a
file? The smallest is about 9 MB

I can take a look, but it's obviously a FFmpeg bug, so not something I can
easily fix in Picard.

For the NIN ghost files, it seems that the problem is the embedded cover
art. I removed block #3, the cover, from the tags which seems to solve the
problem. I do get alot of

[flac @ 0xb3de1b48]FRAME HEADER not here

The cover (for the 1. track at least) is a 600x600 24 bit jpg described as
"Media Rage JPG Image".
